The doctors of the BSMU Clinic performed a unique organ-saving operation, which made it possible to preserve the patient's uterus during placenta ingrowth. Using the technique of temporarily reducing the blood supply to the organ, X-ray surgeons minimized the risk of massive blood loss, which was previously a threat in such cases.
"In this operation, we help our obstetricians-gynecologists: through a puncture of the femoral artery, we enter the vessels supplying the small pelvis and occlude them, that is, we close them with special cylinders. Thus, at the time of cesarean section, when maximum blood loss is expected, we provide the most comfortable conditions for the work of surgeons," Albert Gilemkhanov, doctor of the X-ray surgery department of the BSMU clinic says.
Previously, placenta ingrowth led to the removal of the uterus, but thanks to the introduction of innovative technologies at the BSMU Clinic, it became possible to preserve the woman's reproductive organ. Surgery for placenta ingrowth is performed at a certain period of pregnancy.
"We operate closer to week 37, which is 34-36 weeks - according to our clinical protocols. The patient today has 36 weeks - this is the borderline period when we get a viable fetus. The child adapts independently, and in addition, we give the patient the opportunity to be discharged on 4-5 days, if everything is fine, or on 10-11 days, if the child enters the neonatal pathology department," - the acting head of the department of the BSMU clinic Ilnur Musin comments.
Placental ingrowth is usually detected closer to 20 weeks, but modern ultrasound and MRI methods allow diagnosing the pathology much earlier. This problem occurs more often in women who have a scar on the uterus: the placenta grows so deeply into the altered tissues that it cannot separate on its own during childbirth. Therefore, in such cases, an exclusively cesarean section is indicated. But, unlike the standard operation, the procedure is significantly delayed during ingrowth.
This operation is performed using X-rays, so the entire surgical team puts on a suit made of lead tissue weighing 7-12 kilograms. Surgery lasts about two hours.
It is important to note that the doctors of the BSMU Clinic already have tremendous experience: they carry out such operations every day, providing assistance to more than 200 patients. Moreover, on February 14, 2023, the doctors of the maternity ward of the BSMU Clinic delivered the first patient in the Republic of Bashkortostan, who became pregnant and gave birth to a full-term baby, after organ-preserving surgery for placenta ingrowth.
